Coroner Office Information/Data Request

    Information Request Form

    This form is for OFFICIAL AGENCY REQUESTS or PUBLIC INFORMATION RELEASE requests only. 

    Families are encouraged to call the main office for assistance and updated information at 317-327-4744. 

    Data/information collected and managed by the Marion County Coroner's Office is available upon request with the appropriate authorization as directed in IC 36-2-14.

    Please complete and submit the form for your request. Examples of requests may include, but are not limited to:
     
    • Media releases
    • Autopsy Reports
    • Information requests or Verdict
    • Case files
    • Data sets

    For large information releases (multiple decedents in one request), please attach a document to the form including all applicable information. Once the form is submitted, a staff member will review the request and assign it to the appropriate agency department. 

    ****Some requests may have an associated cost. You will be contacted by a staff member for invoicing and payment prior to document release.****

    All data requests are filled based on availability of staff and case closure. 

    Detailed information on prior year data trends is available on our website in the Annual Report. Current year data is ONLY released on a quarterly basis upon request and following approval from the Coroner or Chief Deputy Coroner.
